Psychotherapy Notes
Psychotherapy notes receive additional protection under HIPAA.
With limited legal exceptions, I will not use or disclose psychotherapy notes without your written authorization.
Exceptions include:
- Using the notes for your treatment.
- Training or supervising mental health professionals.
- Defending myself in legal proceedings initiated by you.
- HIPAA compliance investigations.
- Situations required by law.
- Preventing a serious and imminent threat to health or safety.
- Certain coroner or medical examiner duties.
Marketing
Your Protected Health Information will never be used for marketing purposes without your written authorization.
Sale of Health Information
Connection Matters Colorado will never sell your Protected Health Information.

If you pay for services completely out of pocket, you may request that information related to those services not be shared with your health insurance plan.
Request Confidential Communication
Ask that I contact you in a specific way (for example, by email, cell phone, or at an alternate mailing address). Reasonable requests will be accommodated whenever possible.

Request an electronic or paper copy of your health record (excluding psychotherapy notes). Copies are generally provided within 30 days. A reasonable cost-based fee may apply.
Request an Accounting of Disclosures
Request a list of certain disclosures made during the previous six years that were not related to treatment, payment, healthcare operations, or made with your authorization.
Request an Amendment
If you believe information in your record is inaccurate or incomplete, you may request that it be corrected or supplemented. If your request cannot be granted, you will receive a written explanation.
Receive a Copy of This Notice
You may request a paper or electronic copy of this Notice at any time, even if you previously agreed to receive it electronically.
